The purpose of the observation form is to measure the quality of program implementation. It has been developed for grantees awarded funds from the Teen Pregnancy Prevention Program administered by the Office of Adolescent Health within the Office of the Assistant Secretary for Health. This tool can be used to assess whether further training or changes in staff are needed.
